FIT4MOM FOLSOM LAKE MONTHLY MEMBERSHIP AGREEMENT
AUTHORIZATION FOR PAYMENT
I authorize FIT4MOM Folsom Lake to initiate a charge to the account entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my membership in writing through the FIT4MOM Folsom Lake administrator.
I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Folsom Lake to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.
CANCELLATION POLICY
To cancel this membership, the member must contact the FIT4MOM Folsom Lake administrator at folsomlake@fit4mom.com at least 30 days prior to their next payment due date to request a cancellation, or a $29 early termination fee is applied. Cancellations will be confirmed in writing to the member once they have been processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Folsom Lake franchise. Once a membership is cancelled, if the member wants to re-enroll they will be charged half the normal registration fee. Instructors are not authorized to cancel memberships.
HOLD POLICY
Memberships may be held a maximum of 3 months per hold period. To place a hold on this membership, the member must contact the FIT4MOM Folsom Lake administrator at folsomlake@fit4mom.com at least 10 business days prior to their next payment due date to request a hold be placed on their membership. Holds will be confirmed in writing to the member once they have been processed. Hold starts will coincide with the next date dues is scheduled to be drawn out on and can end on any day of the month. When a hold period is ended, the monthly auto payments will automatically resume on the return date. It is up to the client to keep track of their return date. There will be a $25 processing fee charged to your credit card on file for the hold period. Instructors are not authorized to place a hold on memberships

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
